Rationale for the Referral: An after-the-fact physician attestation statement cannot cure a defective CORF plan of treatment that has not been established and signed by the physician prior to the commencement of services.

Rationale for the Referral: In order for CORF services to be covered by Medicare, all three criteria specified in 42 C.F.R. § 410.105(c)(1)(i) must be satisfied.  Therefore, CORF services must be provided pursuant to a written plan of treatment “established and signed by a physician before treatment is begun.”  42 C.F.R. § 410.105(c)(1)(i).
